Caregiver Support

Helping family caregivers navigate the challenges of caring for a loved one with memory loss, dementia, or other cognitive impairments. When you give, you ensure that caregivers receive expert guidance, respite, and vital resources to ease the caregiving journey.

Care Management

Enabling adults to maintain their independence at home through personalized care coordination and support. This includes our Multipurpose Senior Services Program (MSSP), Enhanced Care Management (ECM), and the Medi-Cal Waiver Program (MCWP), which connect individuals to essential services that promote safety and well-being.

Workforce Development

Strengthening healthcare in our community by training and supporting the next generation of healthcare professionals. Your support helps our initiative to expand access to quality care for medically underserved populations.

Endowment Fund

When you donate to an Endowment Fund, your gift lives forever, earning income year after year. Endowed fund cannot be withdrawn – only the revenue generated by the fund can be used to fund programs.

HPC has established Endowment Funds at local Community Foundations. By directing your donation to one of these Community Foundation’s funds, your gift will benefit residents of that county specifically.

Planned Giving

Planned Giving offers donors a win-win situation. Through Planned Giving, our donors are able to help support our programs and gain substantial income tax and estate tax benefits.

By becoming a Sponsor of our Caregiver University education and training program you can support us to deliver empowering programing for local caregivers. In addition, you or your company will receive high visibility acknowledgement of your generous support.

We manage health care and supportive services for medically frail low-income elders so they can stay safely at home.

We educate and support the health care workforce to effectively address geriatrics and community-based care.
