Helping older adults maintain their independence at home.
The Multipurpose Senior Services Program (MSSP) provides both social and health care management services to assist individuals remain in their own homes and communities. Each year our MSSP team serves over 420 community members so that they may live healthy, active lives.
MSSP provides on-going care coordination, links participants to other needed community services and resources, coordinates with health care providers, and purchases some needed services that are not otherwise available to prevent or delay institutionalization.
A team of health and social service professionals provides each MSSP participant with a complete health and psychosocial assessment to determine needed services. The team then works with the MSSP participant, their physician, family, and others to develop an individualized care plan.

Program Qualifications:
-
Are 65 years of age or older
-
Have appropriate Medi-Cal eligibility
-
Live in Santa Cruz County or Monterey County
-
Willingness to participate in an individualized care plan
